All referrals must be made by a professional working alongside the person wishing to access CAPE Services. To make a professional’s referral into Cape services, you will need to first discuss with your client the referral and how they feel accessing CAPE services can support their Mental Wellbeing in the community alongside other interventions. As one of our health and community partners we ask that you complete our referral enquiry form below. Ella Simpson (Mental Health & Wellbeing Team Leader) and/or Sunny Barnes (Clinical Lead) will then respond to this referral enquiry and contact you for an allocated time to discuss the potential referral, including the needs of the client, any possible risks surrounding the client, and the services CAPE can provide.
Following the referral discussion, if part or all the mental health needs of the individual can be supported by CAPE and they agree that a referral should be made, a full referral form will be sent via email on a secure link and should be completed by the professional alongside the client and submitted to CAPE, inclusive of an up-to-date risk assessment. CAPE team meet weekly at a referrals meeting to look at resource, capacity, risk management and to allocate clients. We will keep the referrer and client always informed on the progress of the referral and when the first meeting can be scheduled, aiming to contact new referrals within 1-2 weeks from acceptance of the referral.
